Wegovy before and after: realistic weight loss results

The short answer

In its main trial (STEP 1), adults on Wegovy (semaglutide 2.4 mg) lost around 15 percent of their body weight on average over about 68 weeks, with results building gradually rather than in the first weeks.

Individual before-and-after results vary widely with starting weight, dose reached, diet, activity, and how long you stay on it. Photos online are selective. A realistic month-by-month timeline is more useful than any single comparison shot.

What the trials actually showed

Wegovy is semaglutide at weight-management doses. In its main obesity trial, STEP 1, adults on the full 2.4 mg dose lost roughly 15 percent of their body weight on average across about 68 weeks. That is the honest anchor for a before-and-after: substantial average loss, but built over more than a year, not in the first month. The dramatic side-by-side photos you see are usually the far end of that range, near the finish line, under good conditions.

A realistic month-by-month picture

  • Month 1: Often little visible change. You are on the low starting dose while your body adjusts, and appetite suppression is just beginning.
  • Months 2 to 3: As the dose climbs, appetite falls more and the trend becomes clearer. This is where many people first see it in clothes.
  • Months 4 to 6: Often the steadiest stretch of loss, with before-and-after differences becoming obvious.
  • Months 6 to 16+: Loss continues but usually slows toward a plateau. The full trial-level results come from staying the course.

Why your results will not match a photo

Before-and-after images vary enormously because the inputs vary: starting weight, the dose you reach and tolerate, protein and overall diet, activity and resistance training (which protects muscle and shapes the result), sleep, and how long you stay on treatment. Photos are also selective, flattered by lighting, posture, and clothing. Comparing your week 6 to someone's month 14 is not fair. Track your own before and after

The most useful comparison is you versus you. Weigh under consistent conditions, take monthly photos in the same light and pose, and log dose alongside weight so you can see how your curve responds to each step-up.
