Why the early "after" is underwhelming

The first month or two is titration: the 2.5 mg and 5 mg starter doses introduce the drug gently and are not meant to drive big losses. So a one-month before-and-after that looks minor is completely normal and says almost nothing about the twelve-month one. The bigger, steadier changes arrive once you reach a therapeutic dose.
