Insomnia is not among the most prominent tirzepatide side effects, but some people on Mounjaro or Zepbound report disturbed sleep, usually indirectly from nighttime nausea, reflux, or blood-sugar dips rather than the drug acting as a stimulant.
The counterpoint: tirzepatide (as Zepbound) is approved to treat obstructive sleep apnea, so for many people it improves sleep quality overall. Persistent insomnia has fixable drivers worth working through.
Does tirzepatide cause insomnia?
Tirzepatide is not a stimulant, and insomnia is not one of its headline side effects. But some people on Mounjaro or Zepbound do report disturbed sleep, especially early on, and it is worth understanding why, because the cause is almost always indirect and addressable rather than the drug keeping you wired.
Why sleep can get disrupted
- Nighttime nausea and reflux. Tirzepatide slows the stomach, so food sits longer, and lying down can bring reflux and queasiness that fragments sleep. Eating a large or late meal makes this worse.
- Blood-sugar dips overnight. Eating much less, especially in the evening, can leave blood sugar low overnight, which can wake you. This matters more if you also take insulin or a sulfonylurea.
- An empty, unsettled stomach. Going to bed on almost no food, which is easy when appetite is gone, leaves some people restless.
- General adjustment. The early weeks of any big physiological change can unsettle sleep, and stress about the process plays a part too.
The upside: tirzepatide and sleep apnea
There is an important counterpoint. Zepbound (tirzepatide) is the first medication FDA-approved to treat moderate-to-severe obstructive sleep apnea in adults with obesity, because losing weight around the airway reduces breathing interruptions during sleep. - Eat earlier and lighter in the evening, so the stomach is not working (or refluxing) at bedtime.
