GLP-1 and metformin together: the workhorse combination

Taking metformin and a GLP-1 together is standard practice, not an interaction risk: the mechanisms complement (liver glucose output down, insulin sensitivity up from metformin; appetite, gastric pace, and glucose-dependent insulin from the GLP-1), and trials ran the combination routinely.
The practical overlaps: both medications front-load GI side effects, so starts and escalations can stack queasiness; and long-term metformin depletes B12, which is worth periodic testing since its fatigue mimics GLP-1 fatigue.
Why this combination is everywhere
Metformin has been diabetes care's first move for decades: cheap, effective, and evidence-rich. GLP-1s arrived as the add-on that also transformed weight, and most of the big diabetes trials (the SURPASS and SUSTAIN programs among them) enrolled participants already on metformin. The pairing is not an off-road experiment; it is the paved road.
Mechanistically they divide the work: metformin reduces the liver's glucose output and improves insulin sensitivity; the GLP-1 quiets appetite, slows the stomach, and releases insulin in proportion to glucose. Will metformin be stopped once the GLP-1 works?
