GLP-1s and B12: the vitamin story, untangled

The short answer

GLP-1 medications do not deplete vitamin B12: no mechanism, no trial signal. The confusion imports from two neighbors: metformin (which genuinely lowers B12 over years) and the compounding era, when pharmacies routinely mixed B12 into semaglutide vials.

B12 still earns attention in specific cases: long-term metformin users, people eating very little animal protein on a suppressed appetite, and anyone whose fatigue outlasts the usual GLP-1 fixes. A level is a cheap test with an easy fix.

Do GLP-1s cause B12 deficiency? No.

There is no mechanism by which GLP-1 receptor agonists deplete B12 and no trial signal suggesting they do. Absorption of B12 happens through stomach acid, intrinsic factor, and the far end of the small intestine; slowing gastric emptying does not dismantle that pathway. Deficiency symptoms (fatigue, brain fog, tingling in hands and feet) overlap heavily with what gets casually blamed on GLP-1s, which is how the attribution jumped medications. The honest accounting: evidence for added benefit was thin to absent, B12's real utility was partly pharmaceutical (a stabilizing, pink-tinting additive that also differentiated products legally and commercially), and harm was unlikely since excess B12 is excreted. When B12 genuinely deserves a check on a GLP-1

  • Long-term metformin use: the guideline case, worth doing on schedule regardless of symptoms.
  • A suppressed appetite that crowded out animal protein: B12 lives in meat, fish, eggs, and dairy; months of minimal intake can drain reserves, especially in already-borderline people. (Plant-based eaters supplement by default, GLP-1 or not.)

Should you just supplement anyway?
