Wegovy, depression and anxiety: what the evidence shows

The short answer
Regulators including the FDA and the European Medicines Agency reviewed reports of depression and suicidal thoughts on GLP-1 drugs and did not find evidence of a causal link. Large studies have not shown an increased risk, and Wegovy is semaglutide, so this applies to it.
That does not make mood changes imaginary. A big calorie deficit, losing food as a coping tool, disrupted sleep, and blood-sugar swings all affect mood. Any new or worsening depression or anxiety, and any thoughts of self-harm, deserve prompt clinical attention.
What regulators found
Reports of depression and suicidal thoughts in people taking GLP-1 medications prompted formal safety reviews. The European Medicines Agency examined the signal and concluded the evidence did not support a causal link, and the FDA's review reached a similar place: no clear evidence these drugs cause suicidal thoughts or actions. Large observational studies have generally not found an increased risk of depression, and some have found the opposite association. Wegovy is semaglutide, so this class-level picture applies directly to it.
The honest framing is: the best available evidence does not show Wegovy causes depression or anxiety, and it is not listed as an established causal effect. But "not proven to cause" is not "nothing to watch," which is why prescribers still take mood changes seriously.
Why mood can still shift on Wegovy
- A big calorie deficit. Eating substantially less for months is a physiological stressor. Low intake, blood-sugar swings, and fatigue all flatten mood independently of any drug mechanism.
- Food was doing a job. For many people eating has been a real coping tool. When appetite and food noise switch off, that tool disappears, sometimes abruptly, and what it was managing can surface as low mood or anxiety.
- Blood-sugar swings. Low or fluctuating blood sugar produces the physical feelings of anxiety: shakiness, a racing heart, sweating.
- Disrupted sleep. Nausea and reflux fragment sleep quietly, and poor sleep reliably worsens both mood and anxiety.
What to do if your mood drops
Take it seriously and tell your prescriber. Do not quietly stop Wegovy on your own, and do not assume it must be the drug. Practical first steps: make sure you are actually eating enough and hitting protein, keep blood sugar steady, protect sleep, and notice whether the dip tracks your dose days or your lowest-intake days. Questions people ask
Does Wegovy cause depression?
Regulator reviews, including by the FDA and EMA, did not find evidence that GLP-1 drugs including semaglutide (Wegovy) cause depression, and large studies have not shown increased risk. Mood changes can still occur for other reasons and should be discussed with your prescriber.
Can Wegovy cause anxiety?
Anxiety is not an established effect of Wegovy, and regulator reviews found no causal link to serious mood effects. Some people still feel more anxious, usually from indirect causes like blood-sugar swings, undereating, disrupted sleep, or losing food as a coping tool.
Why do I feel low or anxious on Wegovy if it isn't causing it?
Common contributors are a large calorie deficit, blood-sugar and energy swings, fragmented sleep from nausea, and losing eating as a way you managed stress. Rapid body change is also emotionally loaded. These are real, addressable, and worth naming.
Should I stop Wegovy if my mood drops?
Talk to your prescriber rather than stopping on your own. They can assess whether the medication, the calorie deficit, sleep, or something else is driving it. If you have thoughts of self-harm, seek help immediately: call or text 988 in the US, or your local emergency number.
