Semaglutide and dizziness: why it happens and how to settle it

The short answer
Dizziness is reported on semaglutide (Wegovy, Ozempic) and is usually indirect: low or swinging blood sugar, dehydration, and a big drop in food intake, rather than a direct drug effect. It is most common early and around dose increases.
Steady meals with some protein and carbohydrate, good hydration, and standing up slowly settle most of it. Dizziness with shakiness and sweating can signal low blood sugar, especially with insulin or a sulfonylurea; severe or persistent dizziness needs a clinician.
Can semaglutide cause dizziness?
Dizziness is reported on semaglutide, most often in the first weeks and around dose increases, but in most cases the drug is not making you dizzy directly. Semaglutide (Wegovy, Ozempic) strongly suppresses appetite, and that sets up several familiar causes of dizziness at once. That is useful, because the causes are mostly fixable.
Why dizziness happens on semaglutide
- Low or swinging blood sugar. Eating much less, and irregularly, can leave blood sugar low, and hypoglycemia causes dizziness, lightheadedness, shakiness, and sweating. This is more likely if you also take insulin or a sulfonylurea.
- Dehydration. Appetite loss usually means drinking less, and mild dehydration lowers blood pressure and causes lightheadedness, especially on standing.
- A big calorie deficit. Sharp undereating leaves you low on fuel, which feels like weakness and dizziness.
- Nausea and inner-ear effects. Feeling queasy can itself come with a woozy, off-balance feeling.
What helps
Aim at the drivers. Do not skip meals just because appetite is low; regular, balanced meals with some protein and carbohydrate keep blood sugar steady. Drink steadily through the day, and add electrolytes if intake has dropped a lot. Stand up slowly from sitting or lying down. Occasional mild lightheadedness that settles with food and fluids is usually manageable. See a clinician for dizziness that is severe, persistent, or recurring, or that comes with fainting, chest pain, a racing or irregular heartbeat, confusion, or one-sided weakness or slurred speech, which need urgent assessment for other causes.
